Professional HealthCare at Home is a full service home health care agency, offering both medical and non-medical home care to families throughout the San Francisco Bay Area. Our service area includes San Francisco as well as Oakland, Berkeley and Alameda County, Walnut Creek, Rossmoor, Concord and Contra Costa County, Daly City, Redwood City and San Mateo County, Sunnyvale, San Jose and Santa Clara County, and Vallejo, Fairfield and Solano County. Services include nursing care, physical and occupational therapy, post-hospital rehabilitation, extended care, wound care, home health aides, medical social workers, medication management, senior care and hospice care.
Home caregivers and personal care assistants provide help with activities of daily living, eldercare, Alzheimer's and dementia care, transportation and companionship. SITE MAP
