Position: RN Case Managers (All Branches)

Position Summary:
The RN Case Manager provides professional nursing services to an assigned group of patients and families in the home setting; responsible for on-going coordination and evaluation of care provided by multiple health care disciplines to ensure patients and families are provided information and access to all required services; promotes self-care, prevents episodes of illness and readmission among at-risk clients, when possible; assists patient and caregivers in determining health requirements and resources to remain in the home or for appropriate referral to placement or hospice; and plans and carries out an appropriate course of treatment to meet patient needs. $2000 sign-on bonus (full-time only)!

Qualifications:
Current California RN license BSN/PHN preferred. Minimum 1 year acute care experience required, recent home care experience strongly preferred. CPR Certified. Current California Drivers License. Must read, write and speak fluent English.
