Position: LVN and/or RNs for 1:1 care (All Locations)

Position Summary:
The Extended Care Team is always in need of LVNs for 1:1 patient care in the home, throughout the Bay Area. The Extended Care LVN provides care in the home setting where nursing situation is stabilized and the initial patient assessment has been completed by an RN; works as a member of the nursing team under the supervision of an RN; the LVN is required to follow a Plan of Care that is initiated by an RN and is responsible for reporting any changes in patient condition or any supplemental orders to the Case Manager or Supervisor; an RN may perform the job duties of the LVN descibed herein.

Qualifications:
Current California LVN License (or RN License if position is being filled by an RN). Minimum 1 year recent acute care experience required. 1 year recent home care experience strongly preferred. CPR Certified. Must read, write and speak fluent English.
